All membership benefits are for active members in good standing during the term of their membership. Membership agreement is required for a 90 (ninety) day minimum period. After a 90 day period membership services will contact you regarding membership continuation which can beextended to a (1) year time period.

If you do not contact membership services, the plan will be continued up to a (1) year period after the initial 3 months of contract. There is no fee to discontinue membership. Membership can be terminated at any time with email cancellation including name, phone number and last 4 digits on credit card used for account.  Cancellation must be made 1 month prior to next billing cycle for cancellation to be placed in effect. Cancellation of membership can be done via phone directly to spa or email myempowercenter@gmail.com. Memberships are non-transferable and non-refundable.

Membership Freeze: A membership may be frozen for a minimum of one month and a maximum of three months at participating locations. Memberships can only be frozen two times in a 12 month period for a maximum of 3 months at a time. A membership must be unfrozen a minimum of 30 days before it can be frozen again. A freeze fee may apply. Members cannot utilize any member benefits during freeze (retail discounts, member benefits). Membership cannot be frozen until after the initial (3) month period. Freezing a membership is completely at the discretion of each individual Spa based on individual circumstances. Empower Med Center must be notified about any changes in address, contact information and/or credit card information. Guests must follow spa rules and regulations. Empower Med Center reserves the right to refuse or discontinue services for any reason. Empower reserves the right to modify/change spa rules, regulations, services and pricing with reasonable notice.

Twenty four hour advance notice is required on any appointment cancellation. No show or no cancellation made within 12 hours of appointment will result in a charge of $25 automatically charged. Empower Med Center is not responsible for lost or stolen property. Age requirements for receiving spa services vary by state and local law and Empower Medical Center strictly complies with such laws. In the event there is not a specific regulation in your area, our policy is as follows:

No person will be denied admission based upon race, religion or appearance. CHILDREN UNDER THE AGE OF 12 SHALL BE STRICTLY PROHIBITED FROM ENTERING THE SPA. No children under the age of 18 will be able to receive any service without the express written consent of their parent or guardian. For children 15 and under (or where required by state law), a parent or guardian MUST remain in the treatment room and for all minor children the parent must remain on premises during services.
